Faith Haven House provides a structured, faith-centered admissions process designed to help us understand each applicant’s situation, confirm program readiness, and prepare residents for a successful transition into the home.

Every person’s situation is different. Our team reviews each applicant carefully and communicates the next step as clearly as possible.

THE ADMISSIONS PATHWAY

Before Welcome Day

Applicants progress sequentially through these nine stages to verify eligibility and fit.

    Initial Contact

    An inquiry is received by phone, email, website form, referral partner, church, family member, or community organization.

  2. APPLICANTSTEP 2

    Applicant Pre-Screen

    Faith Haven House reviews basic eligibility, immediate needs, and general fit for the program through an initial conversation or pre-screen form.

    The public pre-screen is not the full application and should not request highly sensitive personal, medical, legal, or financial information.

  3. APPLICANTSTEP 3

    Complete Required Admissions Documents

    After initial staff follow-up, applicants may be asked to complete required admissions documents through a secure process.

    A criminal record does not automatically disqualify an applicant. Applications are reviewed individually.

    Drug and Alcohol Screening

    Applicants complete required drug and alcohol testing as part of the admissions process and review the program’s sobriety expectations.

    Admissions Interview

    A Faith Haven House admissions interviewer meets with the applicant to better understand motivation, readiness, accountability, goals, and overall fit for a structured living environment.

  7. CLINICALSTEP 7

    Admissions Readiness Review

    When appropriate, a licensed behavioral health professional may provide an admissions-readiness assessment focused on an applicant’s current ability to participate safely and successfully in a structured community living environment.

  8. COMMITTEESTEP 8

    Admissions Committee Review

    The Admissions Committee reviews the completed admissions file and determines the appropriate next step.

    Possible next steps may include admission, admission with conditions, a wait list, a request for additional information, or referral to another level of support.

    Welcome Day Scheduled

    If approved, Faith Haven House contacts the applicant to schedule Welcome Day and prepare for the transition into the program.

  10. SUCCESSARRIVED

    Welcome Day

    Upon review approval, transition to the home begins. You are welcomed into the program with direct support.

Staff confirms that admission has been approved, the resident’s file is complete, a room is prepared, and the initial arrival plan is ready.

Resident Covenant and House Rules

The resident reviews program expectations, house rules, shared-living responsibilities, and the commitment to a safe, structured environment.

Official Program Admission
